Documentation of the care delivered is a fundamental prerequisite for quality, safety and continuity in health services. The patient record is not merely an administrative or legal document\u2014it is a core communication tool among health professionals and a key mechanism for ensuring clinical effectiveness and patient safety.<\/p>

According to the principles of clinical governance and the guidance of international organizations such as the World Health Organization (WHO), ISQua and NHS England, the quality of documentation is directly linked to the quality of care provided and constitutes a central component of systematic Clinical Audits.<\/p>

The Euro-Mediterranean Institute for Quality and Safety in Health Services supports health organizations in conducting clinical documentation audits of patient records, assessing compliance with clinical protocols, regulatory requirements, documentation standards and patient safety principles.<\/p>
