Documentation of the care delivered is a fundamental prerequisite for quality, safety and continuity in health services. The patient record is not merely an administrative or legal document—it is a core communication tool among health professionals and a key mechanism for ensuring clinical effectiveness and patient safety.

According to the principles of clinical governance and the guidance of international organizations such as the World Health Organization (WHO), ISQua and NHS England, the quality of documentation is directly linked to the quality of care provided and constitutes a central component of systematic Clinical Audits.

The Euro-Mediterranean Institute for Quality and Safety in Health Services supports health organizations in conducting clinical documentation audits of patient records, assessing compliance with clinical protocols, regulatory requirements, documentation standards and patient safety principles.

Our approach is based on structured Clinical Audit methodologies, evaluation of representative samples of patient records and development of evidence‑based improvement plans, aiming not only to identify deviations but also to strengthen the quality of clinical practice.

Clinical documentation audit enables organizations to gain an objective view of the actual implementation of care processes and compliance with quality and safety requirements.

Indicative Experience

The Institute has undertaken the development of the first integrated Clinical Audit and Peer Review framework for the Ministry of Health of the Republic of Cyprus, including the execution of dozens of pilot clinical audits and peer reviews at national level.
